Use this Ontario LLC form to register or renew your LLC registration under the Business Names Act every 5 years. Return the completed form to:

There is an $80 fee payable for new registrations and renewals. Pay the fee with a cheque or money order for $80 made out to the Minister of Finance.

This form applies to:

• limited liability partnerships;
• extra-provincial limited liability partnerships; and
• extra-provincial limited liability companies.

Regarding the Ontario address requested in box 4, an Extra-provincial Limited Liability Company may write “Not Applicable.” Please ensure that the mailing address in box 3 includes a street address, as a post office box is not acceptable.

Ontario LLC registration is available only for an LLC that was formed under the laws of another jurisdiction. An LLC entity is an association that is called a “limited liability company.” An LLC grants limited liability to each of its members regarding the liabilities of the association. An LLC is not a corporation.
