Filing an Initial Return for Ontario Corporations
All Ontario corporations must submit this Ontario Corporation Initial Return within 60 days of incorporating, to list the corporation’s officers. The same form must be submitted within 15 days of any change in the listing of the officers and directors in the corporation, their addresses, or the corporation’s address.

This newly revised Notice of Change form 1 also asks for your the official email address for your business so that you can receive:
- Your login
- Filing updates
- Important notifications about the launch of the new corporation registry
How to Fill in the Initial Return
To complete the form, you will need to know the corporation’s legal name, its corporation number, and its date of incorporation. The form has detailed instructions on the first 3 pages.
- In Box 1: Check one of the boxes to indicate whether you are submitting an Initial Return or a Notice of Change request. Don’t check both boxes. The top row in box 1 is for Business Corporations and the bottom row is for Not-For-Profit Corporations.
- In Box 2: Fill in your Ontario corporation number. It’s usually 7 digits and it is on the top right hand corner of your Articles of Incorporation. Sometimes, you will see two zeros before your Ontario corporation number.
- In Box 3: Fill in the Year of Incorporation, Month of Incorporation, and day of the month of your original incorporation.
